The "Bayou-O-Sound" box.

The input is a stereo jack. The outputs are mono, one from the tip, one from the ring.

You can use this box to split the output of a stereo instrument into two different amps.

All connectors are Switchcraft and the cable used is Canare GS-4 mini instrument cable, which is fully shielded.

Please note that the boxes I am using now are 1" x 4" long, 1 inch longer than the box pictured.
